Pharmacist - PRN - On Site

This is an on-site position at the Hospice of Southern West Virginia.


SUMMARY: 

Provides pharmaceutical care, which includes all medication related activities provided directly to the patient with definite outcomes intended to improve the patient’s quality of life.  The Pharmacist must accept personal responsibility and accountability for the patient’s outcomes. 

ESSENTIAL FUNCTIONS:

  • Reviews stock reports for ordering needs.
  • Orders replacement stock based on facility needs.
  • Fills automated dispensing machine.
  • Assures proper medication storage.
  • Destroys unused patient medications as needed, documenting any destruction in compliance with the applicable regulatory requirements.
  • Supports facility P&T committee functions.
  • Provides drug information to professional staff.
  • Responsible for accurate documentation of all work completed.
  • Maintains customer service focus to achieve excellent working relationship with the contracted facilities. 

REQUIRED EDUCATION and/or EXPERIENCE: 

  • Bachelor of Science in Pharmacy or Doctor of Pharmacy degree required.

REQUIRED CERTIFICATION, LICENSURE, and/or REGISTRATIONS:  

  • Pharmacist licensure in the state of West Virginia is required.
  • Maintain active login credentials with wholesaler supplying Hospice of Southern West Virginia is required.
  • Maintain CSOS ordering privileges with Hospice of Southern West Virginia is required.
